Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at Superior Hospice of Northwest Louisiana, LLC. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients and their families.

Key Responsibilities
  • Assess and Coordinate Patient Care: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop and implement individualized care plans to meet those needs.
  • Communicate Effectively: Communicate with patients, families, and healthcare team members to ensure seamless care coordination and to address any concerns or issues that may arise.
  • Manage Patient Records: Maintain accurate and up-to-date patient records, including progress notes, medication lists, and other relevant information.
  • Collaborate with Interdisciplinary Team: Work closely with other healthcare professionals, including physicians, nurses, social workers, and chaplains, to ensure comprehensive and coordinated care.
  • Ensure Compliance with Regulatory Requirements: Ensure that all care is delivered in accordance with relevant laws, regulations, and standards, including HIPAA and OSHA.
  • Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives and participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Education and Experience: Bachelor's degree in Nursing and 2 years of experience in hospice care or a related field.
  • Licensure and Certification: Current RN licensure and certification in hospice care or a related field.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong analytical and problem-solving skills.
