Job Title: Registered Nurse Case Manager

Job Summary:

The Registered Nurse Case Manager will plan and provide individualized nursing care and comprehensive services to home care patients. This role will assist in clinics and other agency programs as assigned by the Nursing Manager.

Key Responsibilities:
  • Assesses patient's health problems and needs and develops plan of care.
  • Provides direct skilled nursing care as per physician orders.
  • Observes patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary action.
  • Document in medical record and prepares other reports as needed.
  • Educates patient and families in self-care and health management.
  • Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
  • Provides case management when appropriate.
  • Attends case conference as requested.
  • Adheres to policies and procedures.
  • Participates in agency quality management and safety activities.
Requirements:
  • Graduate of an accredited school of nursing - required.
  • Current license to practice as a registered nurse in the state of Massachusetts - required.
  • Current CPR required - preferred.
  • One year nursing experience, preferably med-surg - required.
  • Possess valid State of Residence Driver's license and reliable transportation.
