Overview

We are seeking a skilled Home Health Social Worker to join our team at Enhabit Home Health & Hospice. As a national leader in home-based care, we offer a unique opportunity for professionals who want to make a meaningful impact in patient care.

Job Summary

The successful candidate will perform in-home patient visits, coordinate patient care with other healthcare providers, and document accordingly. We also encourage collaboration with community resources to provide the best possible care for our patients.

Responsibilities
  • Perform in-home patient visits to meet the needs of the patient.
  • Contact physicians regarding patient changes and receive verbal orders.
  • Notify physicians and designated branch staff of significant changes in the patient's status.
  • Coordinate patient care with other healthcare providers, branch, and physician; document appropriately.
  • Use community resources to refer patients or act as an intermediary on their behalf.
  • Provide rehabilitative and supportive casework to restore patients to their optimum level of social and health adjustment.
Qualifications

To be successful in this role, you must have:

  • A Licensed Master Social Worker (LMSW) in the state of employment.
  • A Master's degree from a school of social work accredited by the Council on Social Work Education.
  • At least one year of social work experience in a healthcare setting.
  • PREFERRED: Medicare home health or hospice experience.
