Our dedicated Primary Care Practitioners deliver exceptional, all-encompassing medical services to a varied safety-net patient demographic within a collaborative, patient-focused framework. This model emphasizes disease prevention, health promotion, and chronic disease management. The clinical team comprises certified medical assistants, registered nurse care coordinators, clinical pharmacists, health navigators, and behavioral health consultants.

To ensure the provision of high-quality care to patients with intricate needs, our practitioners oversee patient panels ranging from 700 to 1,200 individuals, typically managing around 14 patient consultations daily. After-hours support is provided through a shared call system among the primary care group, supplemented by a nurse triage line that addresses the majority of after-hours inquiries. Our providers also have opportunities to mentor medical residents and students from various institutions.

Community Health Centers of Benton and Linn Counties (CHC)
The CHC, established as a Federally Qualified Health Center (FQHC), serves as the medical home for over 9,000 patients across multiple clinics.
