LPN - Family Medicine Clinician

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Family Medicine team at Essentia Health in Baxter, Minnesota. As an LPN, you will provide direct patient care in the ambulatory care setting, working collaboratively with our team of RNs, physicians, and other providers to deliver high-quality patient care.
Key Responsibilities- Perform routine assessments, including vital signs, patient histories, and symptom observation, to assist in diagnosing and monitoring patient health.
- Support physicians and nurse practitioners during examinations and minor procedures, preparing equipment, assisting with patient positioning, and ensuring proper infection control.
- Administer prescribed medications, including injections and oral drugs, and monitor patients for any adverse reactions or side effects, ensuring accurate documentation.
- Collaborate with healthcare professionals to develop and implement individualized care plans, and assist in coordinating follow-up appointments and referrals.
- Conduct initial patient intake processes, including gathering medical histories, updating personal information, and preparing patients for examination or diagnostic testing.
- Current licensure as a Licensed Practical Nurse (LPN) in the relevant state.
- Ability to work collaboratively as part of a team.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive patient information.
- Competitive hourly rate: $29.32/hour.
