Job Title: LPN - PRN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a PRN (as needed) home health professional. As an LPN with Adoration Home Health and Hospice, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

As a PRN LPN, you will provide high-quality patient care in the comfort of their own homes. Your responsibilities will include:

  • Performing selected nursing procedures within the scope of LPN practice
  • Providing patient and family teaching according to plan outlined by the RN
  • Monitoring patient's vital signs and making specific measurements and observations
  • Monitoring and recording intake and output, reporting significant changes immediately
  • Observing, assessing, and recording patient's physical and mental condition and reporting any changes
  • Administering medications as instructed and as ordered by the physician and instructed by the RN and documenting the patient's file accordingly
  • Attending and participating in multi-disciplinary patient case conferences, staff meetings, and in-service education programs as assigned
  • Cooperating with on-call schedule, weekends, and after-hour visits as necessary
Requirements:

To be successful in this role, you will need:

  • Licensed Practical Nurse (LPN) license in good standing
  • Previous experience in home health, hospice, hospital, or skilled nursing preferred
  • Active CPR certification
  • Familiarity with electronic health records
  • Strong problem-solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
  • Reliable transportation, valid driver's license, and current automobile insurance
