Clinical Case Manager

Arms Acres is seeking a dedicated Clinical Case Manager to enhance our outpatient services. In this position, you will deliver extensive clinical case management support, which encompasses: assessments, individual, group, and family therapy, treatment plan coordination, liaison with referral sources, discharge planning, and engagement with client support networks, including social service initiatives and managed care systems.
KEY RESPONSIBILITIES:
- Conducts thorough evaluations of patient functioning in accordance with facility policies and procedures.
- Facilitates the creation and execution of the multidisciplinary treatment plan; oversees treatment plan reviews; ensures these tasks are completed within designated time frames.
- Records patient progress and personal interventions in case files, adhering to regulatory time constraints.
- Provides individual, group, and family therapy (psycho-education, life skills) as specified in patient treatment plans and as requested by the program director.
- Engages in utilization reviews, generates progress reports, and formulates aftercare plans in collaboration with referring individuals/agencies, EAPs, and managed care/insurance organizations.
- Participates in facility program and quality enhancement initiatives.
- Performs additional duties as assigned.
ESSENTIAL QUALIFICATIONS:
- CASAC-T, CASAC, MSW, LMSW, LCSW, or LMHC credentials.
- Master's Degree in a relevant field.
- Experience in outpatient substance abuse counseling is preferred.
- Bilingual capabilities are preferred.

$23.00-$35.00/ hour
