This role involves providing ongoing services to clients, ensuring their safety and well-being while promoting independence and empowerment.

The ideal candidate will possess strong interpersonal skills, be able to work independently, and demonstrate a commitment to delivering exceptional care.

Salary: $45,000 - $55,000 per year

Job Description:

Key Responsibilities
  • Provide high-level support to service recipients, focusing on meeting their unique needs and goals.
  • Develop and implement individualized service plans to ensure optimal outcomes.
  • Maintain accurate records and documentation, adhering to agency standards.
  • Promote a strengths-based perspective, empowering individuals to reach their full potential.
  • Collaborate with the team to ensure seamless service delivery and effective communication.
  • Stay up-to-date with community resources and services, advocating for clients' needs.
Requirements
  • Must be at least 18 years old.
  • Meet all requirements outlined in 7 AAC 125.010 – 7 AAC 125.199.
  • Pass a Criminal Background Check (unless granted a variance).
  • No prior denials or revocations related to patient services.
  • Ability to assist with ADLs and provide medical-related support.
