About the Job

Aequor Technologies is seeking an experienced Occupational Therapist to join our team in a Skilled Nursing Facility (SNF) setting. As an Occupational Therapist, you will play a vital role in helping patients regain or improve their independence in daily activities.

Job Responsibilities
  • Assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility.
  • Develop and implement individualized treatment plans to enhance fine motor skills, coordination, strength, and mobility.
  • Educate patients and their families on adaptive techniques and the use of assistive devices to enhance safety and independence at home.
  • Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care.
  • Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
  • Assist in the discharge planning process, providing recommendations for continued therapy, home modifications, and support services.
Requirements
  • Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility (SNF) environment.
  • Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team.
  • Proficiency in creating and implementing individualized treatment plans based on patient needs.
  • Active [STATE] Occupational Therapist (OT) License in good standing.
  • A minimum of 2 years of experience as an Occupational Therapist or in a similar role.
  • Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).
  • Experience with EPIC or similar charting systems.
