Supporting Exceptional Patient Care

The role of a Medical Scribe at Oak Street Health is to provide critical support to our primary care providers, enabling them to focus on delivering exceptional patient care.

Scribes collaborate with providers throughout the patient journey, participating in morning huddles to plan the day's visits, observing and documenting patient encounters in real-time, and assisting with post-visit tasks.

As clinical documentation assistants, our Scribes play a vital role in ensuring accurate, specific, and timely documentation. They become experts in our value-based care model and the documentation and care of chronic conditions, including ICD-10 and CPT coding.

By leveraging their expertise, Scribes help providers identify and address care gaps, contributing to improved patient outcomes.

Our Scribes receive comprehensive on-the-job training in clinical workflows, value-based medicine, preventative care for chronic conditions, accurate and specific documentation, population health data streams, and team-based care.
