The Care Management Support Coordinator plays a crucial role in facilitating administrative care management tasks. This includes conducting outreach, handling incoming calls, and coordinating services. The coordinator serves as a primary contact for members, providers, and staff, addressing concerns and documenting member records in accordance with current state and regulatory standards.

Key Responsibilities:

  • Assist members in connecting with various health plans and community resources to ensure they receive exceptional customer service.
  • Utilize knowledge of assigned health plan activities and resources as needed.
  • Act as the first point of contact for member and provider inquiries, clarifying care plan procedures and protocols.
  • Support member onboarding and manage daily administrative tasks, including sending welcome letters and educational materials to foster successful member/provider relationships.
  • Maintain and document non-clinical member records to comply with state and regulatory requirements, providing information to providers as necessary.
  • Understand local benefits and resources, making referrals to address Social Determinants of Health (SDOH) needs.
  • Perform additional duties as assigned.
  • Adhere to all policies and standards.

Qualifications:

  • Must reside in Texas
  • High School Diploma – Required
  • Typing speed of 40 WPM - Required
  • Proficiency in Microsoft Office - Required
  • 1-2 years of experience in telephonic outreach to members/patients - Required
  • Experience in a call center environment - Required
  • Knowledge of healthcare practices - Preferred

Position Details:

  • Schedule: Monday - Friday, 8:00 am - 5:00 pm
  • Compensation: Starting at $25/hr (based on experience) plus benefits.
