Key Responsibilities:
- Provide comprehensive care to clients in their homes, including medication administration, wound care, and treatments.
- Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
- Follow up with and execute doctors' orders, and accurately document observations, interventions, and evaluations.
- Case management and coordination to ensure seamless care delivery.
Requirements:
- A current license as a Registered Nurse in Vermont.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
