We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will provide high-quality patient care in post-acute facilities in the Auburndale, MA area. Responsibilities:
  • Provide patient care as an LPN in post-acute facilities, monitoring residents' physical and mental conditions and tracking vital signs.
  • Perform resident intakes, document medical histories, and track health concerns.
  • Administer and verify medications, tracking their use and effects.
  • Cultivate a comfortable and safe facility for residents.
  • Track medical records and charts.
  • Assist with IVs, immunizations, and phlebotomy procedures.
  • Maintain medical supplies and inventory.
  • Adhere to resident care plans and work with staff to orient and train new colleagues.
Qualifications and Skills:
  • Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of an approved LPN educational program.
  • High school diploma or equivalent.
  • Minimum of 6 months of experience as an LPN or in a related role.
  • Ability to lift a minimum of 50 lbs.
