Key Responsibilities- Conduct thorough psychosocial assessments and screenings to identify patient needs and develop treatment plans.
- Provide individual, group, and family counseling services to patients and families.
- Collaborate with healthcare teams to ensure comprehensive care and coordination of services.
- Develop and implement discharge plans to ensure safe and effective transition of care.
- Monitor patient progress and make adjustments to treatment plans as needed.
- Document patient information and treatment plans in accordance with established protocols.
- Participate in program development and evaluation to ensure high-quality services.
- LMSW required; Epic experience preferred.
- Bachelor of Social Work (BSW) degree or higher.
- Minimum of two years' clinical social work experience, including hospital and psych experience.
