As a Clinical Nurse Coordinator, you will play a vital role in the coordination and delivery of patient care, ensuring that our patients receive the highest level of care and attention.
Key Responsibilities:
- Leadership and Coordination: Demonstrate leadership skills in the coordination and delivery of patient care, ensuring that all aspects of patient care are met.
- Patient Assessment: Systematically assess the health care of patients/families and responses to care rendered, identifying areas for improvement and implementing changes as needed.
- Critical Thinking: Demonstrate critical thinking skills to problem solve and initiate clinical decision making, ensuring that patients receive the best possible care.
- Progress Evaluation: Evaluate patient progress toward health goals and modifies critical path or plan of care accordingly, ensuring that patients receive the best possible outcomes.
- Care Coordination: Develop, coordinate, and provide direct care for specific patient assignments, ensuring that all aspects of patient care are met.
- Safe Environment: Provide a safe environment for patients/families, ensuring that all necessary precautions are taken to prevent harm.
- Communication: Communicate effectively with patient/family and members of the health care team, ensuring that all necessary information is shared and understood.
Qualifications:
- Education: BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service; or combination of education and experience acceptable to Clinical Director of Nursing or designee.
- Certification: Certification preferred in specialty or efforts to achieve certification.
- License: New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN required.
