Job Description:

This full-time position involves conducting comprehensive nutritional assessments, developing personalized dietary plans, and providing education and counseling to patients and their families. The ideal candidate will have a strong background in geriatric nutrition and be able to work collaboratively with healthcare professionals to ensure optimal patient outcomes.

Responsibilities:

1. Nutritional Assessment: Conduct thorough evaluations of patients' dietary needs, taking into account their medical history, current health status, and dietary preferences.
2. Dietary Planning: Develop and implement customized dietary plans for patients, considering their specific health goals and dietary restrictions.
3. Education and Counseling: Provide one-on-one education and counseling to patients and their families, explaining the importance of proper nutrition and dietary changes.
4. Monitoring Progress: Regularly monitor and evaluate patients' progress, adjusting dietary plans as needed to ensure optimal health outcomes.
5. Documentation: Maintain accurate and up-to-date records of patient assessments, dietary plans, and progress notes in compliance with relevant regulations.
6. Collaboration: Collaborate with healthcare professionals, including physicians, nurses, and therapists, to coordinate care and ensure the best possible outcomes for patients.
7. Geriatric Expertise: Utilize your experience with geriatric populations to address the unique nutritional needs and challenges faced by elderly patients.
8. Stay Informed: Stay up-to-date on the latest developments in nutrition science and dietary guidelines to provide evidence-based recommendations.
9. Compliance: Ensure that all dietary recommendations and interventions adhere to applicable laws, regulations, and ethical standards.

Requirements:

• Registered Dietician (RD) credential from the Commission on Dietetic Registration (CDR).
• State licensure as a dietitian/nutritionist in California.
• Bachelor's or Master's degree in Nutrition or a related field.
• Previous experience working with geriatric populations is a plus.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a collaborative healthcare team.
• Excellent organizational skills and attention to detail.
• Proficiency in relevant software and electronic health record systems.
