Job Summary

Key Responsibilities
  • Conduct regular rounds to assess resident condition and implement care plans as directed.
  • Administer medications as prescribed and perform narcotic reconciliation with incoming and outgoing shifts.
  • Oversee and monitor aides to ensure seamless delivery of care.
  • Initiate and complete Incident and Accident Reports as needed.
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Active Licensed Vocational Nurse in the state of Texas in good standing.
  • Experience working in a skilled nursing facility is preferred.
