Key Responsibilities
  • As a Case Manager RN, you will be responsible for coordinating patient care and advocating for their needs while developing effective discharge plans.
  • Your role will involve reviewing patient records and collaborating with families and healthcare teams to determine the best care strategies.
  • Maintaining communication with patients and their families throughout their hospital stay is essential.
Qualifications

Essential Requirements:

  • Current Texas RN License is mandatory.
  • BLS certification is required.
  • A minimum of 2 years of clinical experience is preferred, particularly in inpatient Behavioral Health or home health case management.
  • An Associate Degree in Nursing (ADN) is required.
