As a Home Support Specialist, you will be integral in delivering vital assistance to individuals in their own residences. Collaborating with our multidisciplinary team, you will actively engage in the care planning process to promote the health and comfort of our participants.

Key Responsibilities:

  • Deliver direct personal care and support with daily living activities
  • Assist with additional daily living tasks as required
  • Monitor and record vital signs accurately
  • Help with meal preparation and nutritional support
  • Facilitate safe transfers and provide medical escort when necessary
  • Support the Rehabilitation team with at-home exercise routines
  • Provide translation services if qualified

Requirements:

  • High School Diploma or equivalent
  • Current Basic Life Support (BLS) / First Aid Certification
  • Preferred certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA)
  • Bilingual abilities are a plus
  • Registration with the Department of Social Services Home Care Registry is required
