Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team as a Resource RN. In this role, you will work collaboratively with our interdisciplinary team to provide nursing care to patients and families in their homes. As a Resource RN, you will be responsible for covering nursing needs throughout our service area, providing comprehensive assessments, developing and implementing plans of care, and educating patients and families on end-of-life care and symptom management.

Responsibilities
  • Assess patient and family needs, develop and implement plans of care, and provide education on end-of-life care and symptom management.
  • Provide comprehensive assessments, including physical, emotional, and spiritual evaluations, and update plans of care as needed.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient information, including assessments, interventions, and outcomes, in our electronic health record system.
  • Participate in meetings to plan and evaluate patient/family services and provide feedback to the team.
Requirements
  • ADN or BSN required.
  • Current, unrestricted Registered Nursing License in the state of employment.
  • One year of nursing experience, preferred.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life care.
  • Ability to maintain professional boundaries with patients, families, and co-workers.
