Job Description:

We are seeking a skilled Healthcare Coordinator to join our team in Delaware. As a key member of our care management team, you will be responsible for providing community-based care coordination and management for clients with chronic and complex healthcare needs.

Key Responsibilities:

  • Conduct thorough needs assessments with clients and their families to identify healthcare needs and develop personalized care plans.
  • Collaborate with multidisciplinary teams, including healthcare providers, social workers, and case managers, to ensure seamless care coordination and delivery.
  • Educate clients and their caregivers on available resources, benefits, and services to promote optimal health outcomes.
  • Travel to clients' homes, nursing facilities, and other community-based settings to provide face-to-face care coordination and support.
  • Facilitate transitions between care settings, ensuring smooth handoffs and minimizing disruptions to care.
  • Document all case management activities and interventions in electronic health records, adhering to company and regulatory standards.

Requirements:

  • Current Registered Nurse licensure in the state of Delaware
  • At least 5 years of experience in case management, discharge planning, or related field
  • Proven track record of success in developing and implementing effective care plans
  • Excellent communication and interpersonal skills, with ability to build strong relationships with clients, families, and healthcare providers
  • Ability to work independently and as part of a team, with a focus on delivering exceptional patient care
  • Proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook)
  • Reliable transportation and willingness to travel throughout the assigned territory

Preferred Qualifications:

  • Certified Case Manager (CCM) certification
  • Licensed Bachelor's Social Worker (LBSW) or Licensed Master's Social Worker (LMSW) degree
  • Experience working with geriatric populations, behavioral health, home health, or related fields
  • Medicare and Medicaid experience

Physical Demands:

  • Ability to safely and successfully perform essential job functions consistent with ADA and other federal, state, and local standards
  • Regularly access low and high spaces, conduct repetitive tasks on a computer, and communicate effectively with clients and team members

Salary:$60,000 - $80,000 per annum, depending on experience
