Vertis Therapy is seeking a passionate Physical Therapist to join our team in a consistent PRN capacity at our beautiful Assisted Living facility in Washington DC.
Key Responsibilities:- Evaluate and treat patients in accordance with physician orders, utilizing therapeutic procedures to address physical function deficits and improve independence, safety, and quality of life.
- Manage the delivery of care throughout treatment, delegating appropriate components to Physical Therapist Assistants under supervision.
- Participate in the interdisciplinary team, providing support and information within the area of physical therapy.
- Review patient information and evaluate its impact on assessment and treatment.
- Conduct and document thorough functional assessments, including home/vocational and equipment needs.
- Develop and implement individualized treatment plans based on patient needs, containing achievable functional goals.
- Record patient progress in the patient record, revising treatment plans as needed throughout the treatment process.
- Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy and five years of direct practice experience.
- Current license/certification in the state of practice.
- Prefer minimum one (1) year experience in rehabilitation, supervision, training, and/or experience in a setting serving the same age/type of patient population.
