Job Summary:

We are looking for a Physical Therapist to work in a fast-paced outpatient rehab clinic treating primarily orthopedic and neurologically involved patients with physical therapy diagnoses.

Responsibilities:
  • Provide physical therapy services to patients with various conditions, including orthopedic and neurological disorders.
  • Develop and implement individualized treatment plans to achieve patient goals and improve functional outcomes.
  • Conduct thorough assessments, evaluations, and reassessments to determine patient progress and adjust treatment plans accordingly.
  • Collaborate with healthcare teams to ensure comprehensive patient care and coordinate with other healthcare professionals as needed.
  • Document patient progress, treatment plans, and outcomes in a timely and accurate manner.
Requirements:
  • IL License
  • 1 Year Experience
  • Required skills include experience in outpatient physical therapy, experience treating orthopedic and neurologically involved patients, and experience with electronic health record, preferably Epic.
  • Must also possess excellent communication skills, compassion, and respect for others, teamwork, and flexibility.
  • Preferred: sport medicine, sliding boards, moist heat, phonophoresis, ultrasound, medications, EPIC, BCLS.
