Job Summary

We are seeking a skilled Registered Nurse to join our team at ChristianaCare. As a key member of our Primary Care Practice, you will provide exceptional patient care and support to our patients and providers.

Key Responsibilities
  • Provide phone triage support for patients and ensure appropriate disposition
  • Perform Nurse Visits, including Annual Wellness Visits, Advance Care Planning, and Transitional Care Management calls
  • Conduct point of care tests, EKGs, and other tests as directed by clinicians
  • Assist providers in examinations and procedures
  • Collaborate with team members to provide a smooth and efficient flow of patients
  • Respond to patient crises situations and ensure adequate coverage within the unit
  • Provide patient education and wellness counseling
  • Support the work of LPNs and/or MAs
Requirements
  • A graduate of a state-approved or National League of Nursing accredited School of Nursing
  • RN license or eligibility for licensure in the state of Delaware and Pennsylvania
  • Two years' experience as a RN, physician office experience preferred
  • BSN preferred; ADN considered with agreement of completion of BSN in 3 years
