About the Role

CarePartners is seeking a skilled Registered Nurse Hospice Home Care to join our team. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in need of hospice services.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, focusing on comfort, dignity, and quality of life.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements

To be successful in this role, you will need:

  • A current RN license in the state of North Carolina.
  • At least one year of experience in hospice or home care nursing.
  • Strong assessment and communication skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied schedules, including evenings and weekends.
