">Job Description">">As a Clinical Care Specialist at Equus Workforce Solutions, you will play a crucial role in providing high-quality support to individuals with mental health needs. Your primary responsibilities will include:">
  • ">
  • Working closely with clinical coordinators to ensure that PHQ9 screenings are completed and re-assessed as needed.">
  • Maintaining open communication between supervisors, direct care staff members, family members, guardians, and collateral individuals regarding the individual's status and progress.">
  • Providing a tentative schedule to supervisors of appointments and reporting weekly on crisis intervention and behavioral sessions.">
  • Creating service plans for implementation with members who require clinical mental health care.">
  • Participating in Interdisciplinary Team Meetings and other pertinent meetings to ensure all team members are informed about the individual's progress.">
  • Completing monthly/quarterly progress notes and attending regular meetings with funders for professional development and coaching.">
  • Conducting training and in-service trainings annually to ensure staff competency on program plan implementation and documentation.">
  • Providing follow-up and reporting observations for continued support of the individual's or family's needs.">
  • Ensuring effective record-keeping to ensure information is readily available to interested parties.">
  • Assisting in developing procedures for training materials for staff serving members to enhance programs and ensure compliance with established guidelines.">
  • Focusing on continuous education on behaviorally based interventions and techniques to remain competitive in the field.">
  • Following up and reassessing PHQ9 scores as needed for members who score 10 or higher, including setting safety plans with high-risk individuals.">
  • Performing other related duties to ensure efficient and effective functioning within the work unit.">

    • The ideal candidate should possess a Master's degree in social work, counseling, or a closely related field, such as LMHC, LCSW, or LMSW, and have 1-2 years of experience working in a behavioral health setting.">
    • You should also possess excellent organizational, communication, problem-solving, interpersonal, and customer service skills, as well as flexibility with work schedules and job duties.">
    • Adept problem-solving skills and the ability to work in a fast-paced, stressful environment are essential, along with the ability to handle confidential information professionally.">
    • You must be able to interact with people of all ages in a supportive manner, manage structure and regulatory standards effectively, and utilize Microsoft Word, Excel, and other required computer programs proficiently.">
    • The minimum age requirement is 21 years, and candidates must meet all agency requirements for pre-employment screening as required by Equus Workforce Solutions and state regulations.">
