Key Responsibilities- Case Management: Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Patient Assessment: Conduct comprehensive assessments to identify patient needs and develop strategies to address them.
- Nursing Care: Provide skilled nursing care, including physical assessments, pain management, medication administration, and wound care.
- Education and Support: Teach patients and families about disease management, medication use, and self-care techniques.
- Documentation: Maintain accurate and timely documentation of patient care, including assessments, care plans, and progress notes.
- Team Collaboration: Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
- Supervision: Supervise and delegate tasks to home health aides and other healthcare professionals as needed.
- Education: Graduate of an accredited school of professional nursing.
- Licensure: Current, unencumbered RN license to practice in Oregon.
- Experience: One year of experience in home health, hospice, or a related healthcare setting.
- Skills: Strong assessment and critical thinking skills, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.
The Home Health RN Case Manager will work in a variety of settings, including patients' homes, hospice centers, and other healthcare facilities. The position requires the ability to work independently and as part of a team, with minimal supervision. The employee may be required to work weekends, holidays, or other hours on a rotation basis, as well as participate in on-call rotations.
