Join Our Team as a Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Bennington, Vermont. As a Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to clients in their homes, promoting their independence and well-being.

Key Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans
  • Perform diagnostic tests and measurements to evaluate clients' physical abilities
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions
  • Continually assess and revise care plans to ensure optimal client outcomes
  • Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for independent living

Requirements:
  • A current Physical Therapist license in the state
  • A minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
