About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Kaiser Permanente in Lawrenceville, Georgia. This is an exciting opportunity for someone with experience in patient care coordination and a passion for delivering high-quality healthcare.

Job Summary

The Licensed Practical Nurse I (LPN I) plays a vital role in providing direct and indirect nursing care to patients and their families. Under the supervision of a Registered Nurse or Physician, you will work collaboratively with the healthcare team to ensure that patients receive exceptional care.

  • Key Responsibilities:
  • Prepare patients for office visits by reviewing their medical history and collecting relevant clinical information.
  • Complete Point of Entry (POE) and address care gaps during patient encounters.
  • Prepare rooms and equipment for procedures and assist practitioners as needed.
  • Initiate interventions and document responses to treatment according to established workflows and guidelines.
  • Participate in emergency care as directed by a practitioner or registered nurse.
  • Provide education to patients and offer preparation materials for diagnostic studies according to guidelines.
  • Schedule appointments for patients with lab, radiology, or specialty consultations as ordered by a practitioner.
  • Monitor electronic messaging and emails related to patient or provider requests and manage messages within scope of practice.
  • Participate in departmental or regional quality improvement activities and initiatives.
  • Engage patients, families, and caregivers in the coordination of care.
  • Resolve patient concerns promptly.
Qualifications

To be successful in this role, you will need:

  • A Graduate degree from an accredited nursing (LPN/LVN) program.
  • A valid Practical Nurse License (Georgia) at the time of hire.
  • Basic Life Support certification from the American Heart Association at the time of hire.
