We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Interim HealthCare Inc. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients' families and caregivers to educate them on care plans, medication management, and home safety.
- Monitor patients' progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Communicate effectively with healthcare teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
- Graduate of an accredited Registered Nurse program.
- Active, unrestricted Registered Nurse license in the State of South Carolina or compact multi-state license.
- Minimum of one year of nursing experience, preferably in home healthcare, Med/Surg, or Critical Care.
