Job Overview

Tenet-AZ Region is seeking a highly skilled and experienced Per Diem Social Worker to join our team in Tucson, Arizona. As a Master of Social Work Level II, you will be responsible for facilitating care along a continuum through effective resource coordination.

About the Job

This is an ongoing per diem/prn position with up to 40 hours per week. The shift is 8 hours, days, and the employment type is per diem. You will work closely with patients, families, and caregivers to promote timely throughput, safe discharge, and prevent avoidable readmissions.

Responsibilities
  • Conduct complex psycho-social assessments and interventions to promote optimal health and access to care.
  • Assess patients at risk for readmission and develop transition plans to prevent hospitalization.
  • Collaborate with physicians, office staff, and ancillary departments to ensure seamless care coordination.
  • Lead multi-disciplinary patient care conferences, including Complex Case Review, to discuss patient care plans.
  • Maintain accurate patient demographic and insurance information.
Requirements
  • MSW based on license requirements of the state in which the Tenet Hospital operates.
  • Over 2 years recent acute health care experience preferred.
  • Leadership, communication, and teaching skills required.
  • Knowledge of quality improvement processes and related healthcare practices.
Estimated Salary

$50,000 - $70,000 per year, depending on experience.
