Overview

As a RN Home Care, you will be responsible for providing high-quality patient care to patients of all ages, from infancy to geriatric. You will work closely with an interdisciplinary team to coordinate care and ensure positive patient outcomes.

Responsibilities
  • Provide patient-centered care using critical thinking skills to ensure positive patient outcomes.
  • Coordinate inter-disciplinary care to patients, including nursing, therapy, social work, home care aide, home medical equipment, and home infusion services as needed.
  • Make decisions regarding home health care for patients, ensuring appropriate visit patterns and resource management.
  • Provide education to patients and families on disease management and self-care.
Qualifications
  • Current licensure by the State Board of Nursing in Illinois or Michigan.
  • American Heart Association HealthCare Provider BLS certification.
  • Valid driver's license and reliable transportation.
  • Electronic Medical Record and Microsoft application basic proficiency.
Preferred Qualifications
  • BSN degree.
  • Experience as an RN in a healthcare environment.
