About the Role

We are seeking a dedicated and compassionate individual to fill this critical position. As a Medical Care Coordinator, you will be responsible for facilitating care along a continuum through effective resource coordination. This includes assessing patient needs, identifying potential barriers to care, and developing personalized transition plans.

  • Assess patients for transition needs and identify those at risk for readmission
  • Conduct complex psycho-social assessments and interventions to promote timely throughput and safe discharge
  • Develop and implement transition plans to prevent avoidable readmissions
  • Liaise with patients, families, and caregivers to ensure seamless care coordination
About the Qualifications

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) or a related field
  • At least 2 years of experience in acute hospital settings
  • Current licensure, certification, or registration to practice as a social worker in the state of Massachusetts
