Nurse Practitioner

The Registered Practical Nurse (RPN) is a vital member of the healthcare team at Unity Health Toronto. In this role, you will work collaboratively with Registered Nurses (RNs) and other healthcare professionals to provide high-quality patient care. Your expertise will be essential in assessing, planning, implementing, and evaluating nursing care to meet the physical, psychological, emotional, spiritual, cultural, and social needs of patients and their families.
Key Responsibilities:
• Collaborate with RNs and other healthcare professionals to develop and implement individualized care plans that prioritize patient-centered care.
• Conduct thorough assessments of patients' health needs, identifying priorities, goals, and strategies for interventions that promote optimal outcomes.
• Manage stable and predictable patient situations, using critical thinking and available resources, and seek support from RNs when necessary for evolving patient instability or complex situations.
• Promote patient independence and recovery through encouragement of involvement in activities of daily living, rehabilitation goals, and other activities.
• Maintain accurate and timely clinical documentation, adhering to current College of Nurses of Ontario standards and Providence Healthcare policies.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient-centered care.
Requirements:
• Membership in good standing with the College of Nurses of Ontario and current Medication Certificate (if applicable), meeting College of Nurses Standards of Practice and Providence Healthcare's requirements.
• Current BCLS certification.
• Previous experience working with older adult populations is desirable.
• Strong understanding of major nursing and health issues, as well as the ethical and legislative framework influencing RPN practice.
• Excellent communication and interpersonal skills, both verbal and written.
• Good attendance record and performance history.
