Job Summary

The Licensed Practical Nurse (LPN) will play a vital role in our care team, providing high-quality nursing care and services to our guests. As a skilled care specialist, you will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our guests.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with the healthcare team to develop and implement individualized care plans, ensuring seamless transitions and continuity of care.
  • Supervise and mentor Certified Nursing Assistants (CNAs) and other care team members, promoting a culture of excellence and accountability.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
Requirements
  • Current Licensed Practical Nurse (LPN) licensure in the state
  • 1-3 years of experience in a long-term care setting, preferably in a skilled nursing or rehabilitation environment
  • CPR certification
