Job Summary:

The LPN will work closely with direct support staff to ensure that personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness are properly performed. The role also involves supervising and participating in mealtime and feeding protocols, administering medications, and reporting and documenting medical findings.

Responsibilities:
  • Supervise and participate with direct support staff in tasks related to personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness.
  • Supervise and participate with direct support staff in mealtime and feeding protocols.
  • Perform medication administration as per procedure outline.
  • Report and oversee infection control practices.
  • Ensure adequate basic First Aid supplies are available.
  • Report and document medical findings for RN review.
  • Communicate with families and maintain family contacts.
  • Provide post-operative, routine, or preventative medical treatments and interventions.
  • Obtain baseline and monitor medical information.
  • Ensure ongoing compliance with Federal and State regulations.
  • Utilize appropriate and expected medical charting skills.
  • Attend medical appointments as requested by RN Supervisor.
Qualifications:
  • Current NYS Licensed Practical Nurse Licensure required.
  • AHA or equivalent CPR certification.
  • Experience with people with intellectual and developmental disabilities preferred but not required.
  • Excellent written and verbal communication skills.
  • Ability to handle multiple priorities and work as a member of a team.
