We are seeking a highly skilled Disease Management Program Coordinator to join our team at Kaiser Permanente. As a key member of our healthcare team, you will be responsible for managing the planning, development, implementation, and evaluation of disease management programs for at-risk patients in a behavioral healthcare population.
Key Responsibilities:- Program Development: Collaborate with Chief Physicians, staff physicians, and managers/care providers from multiple disciplines/departments to develop and implement clinical pathways and care management interventions.
- Care Coordination: Coordinate an integrated systematic care management process/program based on clinical pathways, protocols, and guidelines to direct members/patients to appropriate and timely specialty diagnostic and treatment care services.
- Program Evaluation: Analyze program outcomes, identify areas for improvement, and make recommendations for program revisions to ensure effective interaction between clinicians/providers, appropriateness of program care model, quality improvement, and patient/member satisfaction.
- Communication: Act as an organization liaison with relevant community agencies, organizations, and task forces to ensure effective communication and collaboration.
- Experience: Minimum four (4) years of behavioral health case management experience in a relevant chronic disease in an acute medical center, or minimum four (4) years of experience analyzing/improving behavioral health care clinical practices/processes and clinical performance in an acute medical center.
- Education: Masters degree in clinical social work from an institution accredited by the Council on Social Work Education.
- License/Certification: Licensed Clinical Social Worker (California).

This position will support AATC, Bridge Program/Clinic, ETC, and HUB. The work environment is a fast-paced, dynamic setting that requires strong communication and interpersonal skills.
