Job Title: Licensed COTA

At Your Tailor Made Senior Services, we are seeking a highly skilled and compassionate Licensed Certified Occupational Therapist Assistant (COTA) to join our team. As a COTA, you will play a vital role in providing occupational therapy services and procedures in a home health setting.

Key Responsibilities:
  • Provide direct customer care that meets department and professional standards of practice, including customer evaluation and assessment, assisting in development of occupational therapy care plan, customer treatment, family/caregiver training, and discharge planning.
  • Evaluate patients for, recommend, apply, and educate on prosthetic, supportive devices, and equipment as appropriate.
  • Perform all necessary customer care documentation, including assessments, plan of care, progress notes, discharge summaries, and charges for evaluation and treatment.
  • Schedule and maintain assigned customer caseload consistent with department productivity standards.
  • Inform supervising OT if problems arise related to the delivery of customer care or caseload volume.
  • Ensure documentation and billing are timely, accurate, and complete, including evaluations, re-certifications, progress notes, and discharge summaries.
  • Identify need for equipment repair or upgrade to maintain quality of care.
  • Attend and participate in in-service training.
Requirements:
  • Texas Certified Occupational Therapy License
  • Excellent customer service and patient care skills
  • Basic understanding of occupational therapy and medical terminology
  • Active Drivers License
Education and Experience:
  • Associate degree or certification from an occupational therapy program that included course study in anatomy, physiology, psychology, and clinical experience.
Physical Requirements:
  • Prolonged periods of standing and walking
  • Requires lifting, positioning, pushing, and/or transferring patients.
  • Frequent reaching, stooping, bending, kneeling, and crouching.
