The Walker House is seeking a dedicated Care Navigator to help clients navigate the complexities of the healthcare system. As a Care Navigator, you will be responsible for connecting clients with community resources and ensuring they receive the necessary support to achieve their goals.

Your responsibilities will include:

  • Conducting initial and ongoing assessments to determine client needs and strengths;
  • Developing and implementing individualized case management plans to address medical, mental health, housing, employment, and social service needs;
  • Assisting clients in accessing community resources, including healthcare, housing, legal assistance, employment, and educational services;
  • C coordinate care with therapists, medical providers, probation officers, and other stakeholders to ensure a holistic approach to recovery;
  • Monitoring client progress, updating service plans as needed, and providing crisis intervention when necessary.

To be successful in this role, you will need:

  • A Bachelor's degree in social work, psychology, counseling, or a related field (Master's preferred);
  • Meet Kentucky state requirements for Targeted Case Management certification or be eligible to obtain certification;
  • Experience working with individuals with substance use disorders and co-occurring mental health conditions;
  • Knowledge of trauma-informed care, harm reduction, and recovery-oriented practices;
  • Strong understanding of community resources, Medicaid services, and social service systems in Kentucky;
  • Excellent communication, documentation, and organizational skills.
