Job Summary

We are seeking a highly skilled Patient Health Navigator to join our team at SAN JOAQUIN COUNTY HEALTH COMMISSION. As a Patient Health Navigator, you will play a critical role in ensuring that our members receive the highest quality care and support.

Key Responsibilities
  • Conduct Health Risk Assessments: Conduct initial health risk assessments for selected members, triage as appropriate, and provide education on health risks and prevention strategies.
  • Develop Care Plans: Collaborate with primary care physicians and care management staff to develop and implement care plans that meet the unique needs of each member.
  • Coordinate Care Services: Assist members in identifying and accessing community resources, including transportation, DME, and translation services, to ensure seamless care coordination.
  • Monitor Progress: Regularly monitor member progress, identify barriers to care, and develop strategies to overcome them.
  • Document Activities: Maintain accurate and detailed records of member interactions, care plans, and outcomes in our electronic health record system.
Requirements
  • Education and Experience: High school diploma or equivalent required; at least two years of experience working as a community health advocate, patient advocate, or patient coordinator preferred.
  • Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, basic knowledge of medical terminology, and proficiency in Microsoft Office applications.
Preferred Qualifications
  • Associate's Degree: Associate's degree in a related field, such as healthcare administration or social work, preferred.
  • Experience in Managed Care: Experience working in a managed care setting, including knowledge of Medi-Cal and other government-sponsored programs, preferred.
