Job Title: RN Case Manager

Pikes Peak Hospice and Palliative Care is seeking an experienced Registered Nurse Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality patient care and coordinating with our interdisciplinary team to ensure seamless delivery of services.

Job Summary:

The RN Case Manager will be responsible for overseeing and coordinating patient and family care in accordance with hospice philosophy and delivery of care. This includes providing skilled nursing, including intervention, documentation, supportive care, and assessments for assigned patients.

Key Responsibilities:
  • Develop and oversee the care plan with patient and family/caregiver, primary physician, and members of the IDT/IDG.
  • Coordinate patient care with the physician, NP, and all members of the Interdisciplinary Team through ongoing communication and appraisal of patient's changing condition.
  • Ensure the educational needs of the patient and family/caregiver are met in order to promote optimal care.
  • Provide continuous assessment and monitoring of the physical, emotional, and spiritual needs of the patient.
  • Perform complex telephone interviews and triages as appropriate.
  • Evaluate the appropriate time to coordinate transfer of patient from home hospice to inpatient care.
  • Supervise Certified Nurse Aides (CNA); delegate specific tasks per agency policy.
  • Participate in related services, such as community education, team training, staff/board committees, and orientation of employees.
Requirements:
  • Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
  • Minimum Experience: 1-2 years experience in hospice/palliative care/home health, medical surgical, public health, oncology, acute care, or home health.
  • Required License: Current Colorado RN license. Current CPR certification. Valid Colorado state driver's license and proof of auto insurance.
  • Required Travel: This position requires daily travel.
