We seek a skilled and compassionate RN to join our team as a Per Diem Hospice Caregiver. As a key member of our multidisciplinary team, you will contribute to the delivery of high-quality, patient-centered care in a dynamic and supportive environment. This opportunity allows you to apply your clinical expertise, empathy, and problem-solving skills to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
Respond to patient calls and inquiries, conduct home visits as required, develop and update patient plans of care, communicate effectively with RN case managers, and maintain accurate documentation of patient interactions and outcomes. Demonstrated proficiency in electronic medical records systems and Microsoft Office applications is essential. Strong organizational and time management skills, along with excellent communication and interpersonal skills, are also required for success in this role.
