Pediatric Services Coordinator Job Description

Honor Community Health is a 501c3 Federally Qualified Health Center dedicated to providing comprehensive, integrated primary, behavioral health, and dental care to the underserved of Oakland County. We are seeking a highly motivated and compassionate Pediatric Services Coordinator to join our team.

Job Summary

The Pediatric Services Coordinator will provide resources and coordination care for pediatric patients, ages 0 – 5, and their families. This role requires tracking and follow-up of pediatric care, ensuring details are documented in the Electronic Health Record and population health tool. The ideal candidate will have a strong background in pediatric health, early childhood development, and family health.

Responsibilities
  • Provide resources and coordination care for pediatric patients and their families
  • Track and follow-up of pediatric care, ensuring details are documented in the Electronic Health Record and population health tool
  • Develop reports, data analysis, and follow-up to meet program requirements and improve quality outcomes
  • Collaborate with the Maternal Child Health care team to provide quality care to pediatric patients and their families
Requirements
  • BSN registered nurse, or ADN registered nurse with bachelor's degree in public health, social sciences, human services, or other related field required
  • Two years of experience directly related to the duties and responsibilities specified is preferred
  • Must be a registered nurse in the state of Michigan or willing to achieve licensure within three months of employment
  • Certified Lactation Counselor training must be completed within one year of employment
  • Must have knowledge and experience in working with the Microsoft Suite (e.g., Excel, Word, PowerPoint)
  • Experience with Electronic Health Records and documentation highly preferred
  • Experience working in maternal child health preferred
  • Additional language fluency in Spanish highly preferred
