LPN (Continuous Care) LifePath Hospice 7a-7p - Nursing Professional

Chapters Health System is seeking a skilled LPN (Continuous Care) LifePath Hospice 7a-7p to join our team. As a nursing professional, you will provide high-quality patient care and support to patients and their families in a compassionate and respectful manner.
Key Responsibilities- Provide nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
- Work under the direction of an RN to provide nursing tasks, including Continuous Care, dressing changes, and catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
- Demonstrate clinical skills regarding application of Company policies and procedures, such as medication administration, symptom management, and patient/family relationships.
- Observe and report signs of adverse reactions and responses to treatments to the RN.
- Perform venipuncture for blood specimens and other duties as assigned.
- A high school diploma or G.E.D. or an equivalent combination of education and experience.
- Current LPN licensure in the state of Florida or current licensure from another state plus a temporary Florida permit.
- Minimum of one (1) year nursing experience in an in-patient, hospice, or home health setting.
- Mobile Driver - Valid driver's license and automobile insurance per Company policy.
- Reliable transportation to meet visit schedule.
- Ability to communicate (written and verbal) in English.
- Flexible schedule to meet the needs of patients.
- Ability to use equipment with visual and auditory mechanisms.
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
- Satisfactorily complete competency requirements for this position.
This position requires the ability to see, hear, talk, walk, use hands to finger, handle or feel, stand, sit, reach with hands/arms, bend, balance, lift, pull, push, stoop/crouch, kneel, and climb stairs.
