Physical Therapist

We are seeking a skilled Physical Therapist to join our team at HealthPRO Heritage. As a Physical Therapist, you will play a vital role in evaluating and treating residents, under the orders of a physician, to restore loss of muscle strength, coordination, and body alignment, relieve pain, and improve movement and function.
Responsibilities- Conduct thorough functional assessments of residents, utilizing standardized procedures, and evaluate the impact of available resident information on the assessment and treatment process.
- Develop and implement individualized treatment plans based on resident needs, containing achievable functional goals, and revise plans as needed throughout the treatment process.
- Comply with evaluation, treatment, and documentation guidelines, and maintain timely and accurate department records.
- Instruct Assistants in treatment procedures and provide close supervision while they work with residents.
- Enter progress notes in resident records following each treatment session and document the course of treatment in a clear, concise discharge summary.
- Orient residents to program services and treatment procedures, inform them of potential risks during procedures, and work with nursing staff, residents, and families to maximize resident use of functional abilities.
- Assist in the referral process when a resident requires additional services following discharge, maintain current knowledge of community resources, and participate in the quality improvement process.
- Adhere to facility policies and procedures, participate in facility quality improvement and safety programs, and perform other duties as assigned to provide quality care to residents.
- Bachelor's or Master's degree in Physical Therapy.
- Current license in the state of practice.
- Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population served by this facility.
- Certification, if applicable, and demonstrated competency in identified areas of specialization.
- Excellent oral and written communication and interpersonal skills.
- Demonstrated competency in applying the principles, methods, materials, and equipment used in physical therapy.
- Ability to assess resident needs and develop and implement a comprehensive plan of care.
- Knowledge of accreditation standards and compliance requirements.
The ideal candidate will possess excellent communication and interpersonal skills, be able to work independently, and demonstrate a commitment to providing high-quality care to residents. The Physical Therapist must be able to lift up to 50 pounds independently and may be required to lift over 50 pounds with assistance. The ability to stand for prolonged periods, push, and reach is also required.
