- Coordinate Patient Care: Coordinate patient care to ensure that patients receive the necessary services and support to meet their needs.
- Develop and Implement Care Plans: Develop and implement individualized care plans to meet the unique needs of each patient.
- Communicate with Physicians and Other Healthcare Providers: Communicate with physicians and other healthcare providers to ensure that patients receive the necessary care and services.
- Manage and Document Patient Information: Manage and document patient information to ensure that accurate and up-to-date information is available to all members of the healthcare team.
- Participate in Quality Improvement Initiatives: Participate in quality improvement initiatives to ensure that patient care is delivered in a safe and effective manner.
- Current RN Licensure: Current RN licensure in the state of practice.
- CPR Certification: Current CPR certification required.
- Driver's License and Vehicle Insurance: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
