Job Description

At PT Plus, we are seeking a skilled Patient Care Coordinator to join our team. This role is responsible for improving the patient experience, converting consumers to patients, and ensuring proper patient case setup.

Key Responsibilities:

Medical Receptionist:

  • Provide general knowledge regarding clinical skills, services, and programs to patients.
  • Greet patients upon arrival, assist with questions, and take payments.
  • Ensure patient paperwork is completed, scanned into EMR same day, and shredded upon completion.
  • Maintain a neat clinic environment through routine, light cleaning.
  • Manage inventory stock for medical, retail, and office supplies.
  • Maintain schedule density for clinics via processes provided.

Patient Care Coordination:

  • Manage central phones, check messages, and return phone calls within 24 hours/1 business day.
  • Manage incoming central faxes, reaching out to referrals within 24 hours/1 business day.
  • Daily Review of EMR Task list.
  • Perform a benefit check for rehab services and educate patients regarding fees for their services.
  • Complete Insurance Authorization process and forms as needed.
  • Maintain coverage throughout the entire POC, communicate with patient and clinical staff regarding interruptions or barriers to patient authorization, including when max benefits have been reached.

Results/Goals:

  • Individual KPIs:
    • IE Patient Paperwork On File: > 90% completion.
    • Follow Up Visits/POC S.

A personable individual who is passionate about patient care and helping others. You will be the first person our patients interact with, improving ease of access to care at PT Plus clinics.
