\Compensation: $85,000 - $115,000 per year, depending on experience
\About the Job: We are seeking an experienced Physical Therapist to join our team in Concord, NH. As a home health Physical Therapist, you will be responsible for providing one-on-one therapy sessions to adult and geriatric clients in their homes. You will work closely with a multidisciplinary team to develop and implement individualized care plans that promote patient independence and well-being.
\Responsibilities:

- Making home visits to clients in designated geographic territories\
- Performing diagnostic tests and measurements, such as mobility assessments, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments\
- Implementing integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents\
- Developing and implementing appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions\
- Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate\
- Educating and instructing clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the client's health, safety, and independent living\
- Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided\
- Working independently and managing time effectively\
- Providing excellent customer service and communication skills\
- Maintaining accurate records and reporting results\
- Collaborating with healthcare professionals to ensure continuity of care\
- Prioritizing tasks and managing multiple responsibilities\
