We are seeking a highly skilled and compassionate Bilingual Nurse Case Manager to join our team at Paradigm. As a Bilingual Nurse Case Manager, you will be responsible for providing medical case management services to injured workers, including assessment, planning, coordination, implementation, and evaluation of care.

Key Responsibilities:
  • Conduct thorough assessments of injured workers to identify their medical needs and develop individualized care plans.
  • Collaborate with healthcare providers, insurance carriers, and other stakeholders to ensure seamless care coordination.
  • Provide education and support to injured workers and their families to promote optimal recovery and return to work.
  • Monitor progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
  • Current, unencumbered Registered Nurse (RN) license in Arizona.
  • Experience in case management of workers' compensation cases, preferred.
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred.
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal).
  • Ability to operate autonomously with minimal oversight.
  • Skilled at patient education.
  • Valid driver's license, and good driving history.
  • Bilingual (Spanish).
