Job Summary

We are seeking a skilled and compassionate Travel Palliative RN to join our team. As a Palliative Care Nurse, you will work with patients, families, and healthcare professionals to provide and coordinate high-quality end-of-life care. Your primary role will involve in-person and telephonic assessments, education, and support for patients and families, as well as collaboration with our interdisciplinary team.

Key Responsibilities
  • Conduct in-person and telephonic RN assessments and screenings
  • Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs
  • Consult with and educate patients/families regarding disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs
  • Communicate findings to the PCP or AIM Palliative NP and coordinate care with palliative team members
  • Support hospice referral as appropriate and document timely clinical records
  • Develop and implement individualized palliative care plans for each patient
Requirements
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
  • Additional experience in hospital settings, med-surg, PACU, float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing a plus
  • Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel
  • Fluent in electronic medical record documentation
  • Strong compassionate phone skills and ability to work independently
